Adding Pages to Passport
Frequent travelers may require the addition of pages to their U.S. passport. This is a same-day turnaround service. To receive additional pages, fill out form DS-4085, Application for Additional Visa Pages, which can also be picked up in hard copy at the American Citizen Services (ACS) unit. On the online form, click “Print a blank form DS-4085.”
How to request an appointment
Applications for Visa pages are by appointment only to ensure that you are prepared with the necessary documents. Please bring your passport and completed DS-4085 to the ACS unit during your appointment.
You may apply in person, or have someone else bring your signed application with your passport. This service will cost $82, which is payable at the consular section's cashier window in cash or by credit card (Mastercard, Visa, Diner's Club, Discovery, or American Express).
