ADDITIONAL PAGES FOR A U.S. PASSPORT

Frequent travelers may require the addition of pages to their U.S. passport.  To receive additional pages, fill out form DS-4085 Application for Additional Visa Pages, which can be picked up in hard copy at the American Citizen Services (ACS) unit.

Please bring your passport and completed DS-4085, to the ACS unit during our hours of operation.  You may apply in person, or have someone else bring your signed application with your passport.  There is no charge for adding pages to your passport.
