Requirements to Apply to be Selected as a Representative Payee for a Civil Service Annuitant or Beneficiary
- Retirement card or any document or correspondence that reflects the annuitant’s or the beneficiary’s Civil Service Claim Number (CSA/CSF).
- If the beneficiary is a minor, and you are not the biological parent, , you should bring the Spanish original document issued by the “Family or Minor Court” awarding the “Custody and Guardianship of the minor child’s estate.” An English translation from a certified translator is required.
- For an adult disabled child or annuitant you should bring an updated medical certification. It should include the patient’s medical condition, date the patient was first treated, the diagnosis, prognosis, and medication(s) if any. If the medical certification is issued in Spanish, the English translation from a certified translator is required. It should also state whether the beneficiary is incapable of handling his/her own funds.
- The applicant should bring his/her cedula.
